What is tramadol?
Tramadol is a powerful painkiller that belongs to the class of medicines called opioids (morphine derivatives). The active ingredient tramadol works by blocking the transmission of pain signals in the central nervous system. This helps to reduce moderate to severe pain. Tramadol is not as strong as morphine, but it is a much more powerful painkiller than paracetamol.
What is tramadol used for?
Doctors prescribe tramadol when other analgesics that are not as strong do not have the desired effect. It can be used to treat moderate to severe pain from:
- An operation or injury;
- Nerve pain;
- Tramadol is also used to treat pain caused by other factors.
How to use tramadol
Tramadol is available as delayed release tablets and capsules or oral drops. The capsules and tablets should be swallowed whole with plenty of liquid. They begin to take effect after about an hour and provide approximately 12 hours of relief.
Tramadol drops can be taken with water. You can also squeeze the drops onto a sugar cube and let it dissolve in your mouth. Tramadol drops reduce pain fast. They take less than an hour to become effective and last for about six hours.
Strength, daily dose and duration must be determined by a doctor. Always follow the prescription exactly – never take more or less than your doctor recommends. The general recommended dose is:
Tramadol Ratio drops (100 mg/ml):
- Adults and children aged 14 and over: 20 to 40 drops, three to four times a day.
Tramadol Ratio Retard tablets and capsules (50 mg, 100 mg, 150 mg or 200 mg):
- Adults and children aged 14 and over: one tablet or capsule twice daily.
The maximum dose of tramadol is 400 mg daily. Orders for more than 400 mg, with a maximum of 30 days, will be rejected by doctors and pharmacies.

Tramadol can cause side effects, including:
- Drowsiness and sleepiness;
- Nausea and vomiting;
Most side effects are usually only temporary and will disappear once the body adjusts to the use of the medication. See the package leaflet for a complete list of possible side effects.
When not to use tramadol
Tramadol is not suitable for everyone and should not be used if:
- you are allergic to tramadol or any other ingredient in this medicine;
- you are pregnant;
- you are under the influence of (intoxicated with) alcohol, sleeping tablets or the medicines or substances that work via the central nervous system;
- you are or have recently been treated with antidepressants (MAO inhibitors).
Tramadol interacts with other medicines, such as hypericum perforatum (St John’s wort) and some medicines for cardiac arrhythmia. It is therefore important to tell the doctor what other medicines you are taking when you order.

Tramadol is not suitable for use during pregnancy. Consult your doctor on the use of this painkiller if you are breastfeeding.
Tramadol may cause you to feel sleepy or drowsy and can slow down your reaction time. Stay out of traffic and do not operate dangerous machinery if you experience these side effects.
You should avoid drinking alcohol while taking this medicine as this will increase the chance of you feeling drowsy or sleepy.
It is not allowed to order more than 400 mg Tramadol a day, with a max. of 30 days, per order. These orders will be disapproved by the doctor and pharmacy.

Tramadol works great
I take three 50 mg capsules a day. I find tramadol very effective for pain in my lower back and legs. So you could say I’m very happy with this painkiller. My own GP prescribed diclofenac 25 mg, but this didn’t give me any relief. I always went to bed in pain, which prevented me from sleeping. The pain was flat out unbearable.
